Quick Order COD Form — Cash on Delivery order forms for Shopify
Last updated: August 11, 2026
What this app does
Quick Order COD Form lets you add a fully customizable Cash on Delivery (COD) order form to your Shopify store. Customers can place orders — paying when their order is delivered — directly from your Home, Collection, Product, and Cart pages, without needing an online payment method.
The app includes tools to build and style the order form, protect your store from fraudulent COD orders, boost order value with upsells and downsells, and configure country-specific shipping and fees.
App navigation
Inside your Shopify admin, under Apps → Quick Order COD Form, you'll find the following sections in the left-hand menu:
| Section | Purpose |
|---|---|
| Home | App dashboard / overview |
| Form Builder | Design your order button and order form |
| Fraud Protection | Block risky orders and configure partial payments |
| Upsells & Downsells | Add offers to increase order value and recover lost sales |

Form Builder Core
Quick Order COD Form → Form Builder
The Form Builder has two modes, selected via tabs at the top: Buy Button and Form.
Buy Button
Controls how the order button itself looks on your storefront (title, subtitle, colors, icon, animation, sticky behavior). Changes appear live in the Preview panel on the right, shown on a sample product page.
- Button title / subtitle: the main label (e.g., "Order Now - Cash on Delivery") and an optional subtitle (e.g., "Pay when you receive your order"), each with independent font size, weight, and alignment.
- Animation: choose an animation style and set a repetition interval (in seconds) to draw attention to the button.
- Button icon: toggle an icon (e.g., a cart icon) and adjust its size.
- Sticky button: enable the button to stay fixed on screen for Mobile only, Desktop, or both. Sticky buttons are only visible on the product page.
- Colors: set background color, border color, icon color, and shadow color independently using hex values.
- Layout: adjust button padding, shadow intensity, border width, and border radius using sliders.
- Use Reset to default at any time to revert all button styling.
Form
Controls the layout and content of the order form itself, which can be displayed as a Popup Form (opens over the page) — the dropdown at top-right lets you choose the display mode. The left panel lists every field on the form, in the order they'll appear:
- Form Style / Field Style: global styling controls for the form container and input fields.
- Product Card: shows the product image, title, variant, quantity, and price (with any compare-at price struck through).
- Discount Code: lets customers enter and apply a discount code; applied codes appear as removable tags, and the order summary (Subtotal, Delivery Fee, Discount, Total) updates automatically.
- Payment details: displays the order totals and "Total (Pay on Delivery)" amount.
- Shipping Method: radio-button list of available shipping rates (configured in Settings → Shipping rates).
- Shipping Details Title, First Name, Last Name, Phone Number, Email Address, Country, State, City, Shipping Address, Postal Code: the customer's delivery details. Required fields are marked with a red asterisk. Country selection dynamically updates the State/Province dropdown.
- Upsell checkbox: displays any active upsell offer (see Upsells) as a checkbox within the form.
- WhatsApp Order Button: optional button allowing customers to send order details via WhatsApp.
- Complete Order Button: the final submission button that places the COD order.
Each field has an eye icon to show/hide it on the live form, and a pencil icon to edit its label, styling, or behavior. Use Add more fields at the bottom of the list to add custom fields. Click Save at the top to publish your changes.
Fraud Protection Security
Quick Order COD Form → Fraud Protection
This section has two tabs: Blocking and Partial Payment.
Blocking
- Repeat order detection: when enabled, customers are identified by IP address. If the same IP address tries to place another order within a configured Time period and Time unit (e.g., 24 hours), the new order is automatically blocked.
- Quantity limit per order: optionally block orders that exceed a maximum quantity, to prevent bulk fraudulent orders.
- Block by IP address: maintain a list of blocked IP addresses (one per line, up to 10,000 characters). Any order attempted from a listed IP is blocked automatically.
- Block by phone number: block orders from specific phone numbers.
- Block by email address: block orders from specific email addresses.
Partial Payment
When Enable partial payment for COD orders is turned on, customers must pay a portion of their order upfront (via your store's normal payment methods) before the COD order is confirmed — reducing fake or abandoned orders.
- Payment type: choose Percentage (e.g., 50% of order total) or a fixed amount.
- Percentage / amount: set the exact value customers must pay upfront.
Upsells & Downsells Revenue
Quick Order COD Form → Upsells & Downsells
This section lets you increase average order value (upsells) and recover customers who are about to abandon the form (downsells).
Upsells
An upsell appears as a checkbox add-on directly inside the order form (e.g., "Add warranty for just $0.00"), letting customers add an extra product or service to their order with one click.
- Upsell Name: internal reference name, only visible to you.
- Apply this upsell for: choose All products, Specific products, or Except products.
- Offer title / Offer price: the label and price shown to the customer. You can use variables
{{title}}and{{price}}inside the description text. - Description: longer explanation of what the upsell includes.
- Styling: background color, tick color, border color, border radius, and border width for the checkbox card.
- Select by default: toggle whether the upsell checkbox is pre-checked when the form loads.
- Enabled: toggle the upsell on/off without deleting it.
All upsells are listed in a table showing Name, Type, Selected by Default, Updated at, Status, and Actions (edit/delete), with a status toggle to enable or disable each one instantly.
Downsells
A downsell is a one-time discount popup that appears when a customer tries to close the order form without completing it — offering a last-chance discount to recover the sale (e.g., "Wait! Special Offer Just For You — Get Extra 10% Off").
- Downsell Name: internal reference name, only visible to you.
- Discount type / value: Percentage or fixed amount off the order.
- Title / Subtitle: the headline and supporting text shown in the popup, each with independent font size and weight.
- Discount badge: the large highlighted discount value (e.g., "10%"), with configurable font size, weight, text color, badge color, and badge size.
- Order button label: the call-to-action button (e.g., "Complete Order"), with font size, weight, background color, and border radius controls.
- Close button label: the dismiss link (e.g., "No thanks") shown below the order button.
Downsells are listed in a table with Name, Type, Updated at, Status, and Actions, with the same enable/disable toggle as upsells.
Settings Configuration
Quick Order COD Form → Settings
Settings are organized into four tabs on the left: Visibility, General, Country, and Shipping rates.
Visibility
- Form/button placement: choose which pages show the COD button/form — Home page, Collection page, Product page, and/or Cart page & Slide cart.
- Product page options:
- Hide the standard "Add to cart" button.
- Hide the standard "Buy now" button.
- When click on COD button: choose whether clicking COD buys only the current product, or the current product plus any items already in the cart.
- Enable COD orders for: All products, Specific products, or Except products.
- Restrict COD by price range: when enabled, the form or COD button is hidden if the order total falls outside your allowed price range.
General
- Enable cash on delivery fee: add an extra fee to COD orders as a Fixed amount or Percentage, with a custom COD fee value and Fee name (e.g., "COD Fee") shown to the customer.
- "Powered by" badge: optionally show a "Powered by Quick Order COD Form" badge below the form.
- Disable all discounts: optionally turn off the discount code field on the COD form entirely.
- Abandoned cart recovery:
- When enabled, if a shopper leaves before completing checkout, a draft order is automatically created.
- Draft orders appear under Orders → Drafts and are tagged
abandoned_order. - Contact the shopper by phone or email to recover the sale.
- Optionally restrict this to only create a draft when the shopper left contact info (phone or email). When enabled, the Partial Payment COD form won't trigger abandoned cart drafts.
Country
- Countries to offer COD: search and select which countries can use the COD form. If no country is selected, COD is offered in all countries by default.
- Enable multiple countries: toggle to allow selecting more than one country.
- State field type: choose between a Dropdown (predefined states based on the selected country) or a free-text Text field for customers to type their state/province manually. A live preview shows both formats side by side.
Shipping rates
- View all configured rates (e.g., "Standard Shipping — Flat rate: $5.00") with edit and delete controls.
- Click Add rate to create a new shipping rate:
- Rate name: label shown to customers (e.g., "Premium Shipping").
- Rate type: Flat rate or Weight-based.
- For weight-based rates: set Minimum and Maximum weight (kg) and the Price for that range. Use the
+icon to add multiple weight tiers, or the trash icon to remove one. - A live Preview shows exactly how the rate will appear to customers.
- Apply this rate only to specific countries: optionally restrict a rate to selected countries only.
- Click Save to publish the new rate, or Cancel to discard it.
